International students must present proof of insurance coverage along with the required Health History and Immunization form to the Student Health Services. Policy information must be presented in English and meet the benefit requirements established by the Florida Board of Governors.

**NOTE: Student Health Services only accepts the University sponsored insurance for payment at this time. Students with insurance from other carriers, may request a completed claim form (HCFA 1500) to submit to their insurance provider for reimbursement of covered services. Students enrolled in the FAMU SHIP, will not need to complete a reimbursement form.
